If you wish to apply for Provisional Licensing, Ministerial Licensing, or Ordination status through AFCM, please download and complete the application form, include your dues payable to AFCM, and mail to the address listed below:

When your application is approved for credentials, you are automatically a registrant.
AFCM also offers REGISTRATION ONLY status to those who currently hold credentials with another organization, but wish to be associated with AFCM. If you are applying for this type of registration, a copy of your current credentials must accompany your AFCM application.
A completed application, AFCM dues, and two recommendation forms must be returned to this office before we can begin to process your application. ONE OF THE FORMS MUST BE GIVEN TO YOUR PASTOR.
Renewal applications and fees are due December 31 on an annual basis. All renewals received after December 31 will be charged an additional $25.00 late fee. RENEWAL APPLICATIONS WILL NOT BE ACCEPTED AFTER JANUARY 31. You must reapply for registration after this date. Registrations are as follows:
- Affiliate Only Status: $75.00
- Individual Provisional or Ministerial Licensing or Ordination: $100.00
AN APPLICATION FEE OF $25.00 IS REQUIRED TO PROCESS YOUR APPLICATION. THIS FEE IS NONREFUNDABLE. WHEN AN APPLICANT IS DECLINED APPROVAL ON HIS/HER APPLICATION OR WITHDRAWS HIS/HER APPLICATION FOR ANY REASON BEFORE PROCESSING IS COMPLETED, THIS FEE WILL NOT BE REFUNDED TO THE APPLICANT.
