Registration Renewals
How do I renew my registration?
The best way to renew your registration is to edit and complete the pre-filled form that was mailed to you at the beginning of November. If you have misplaced it, either request another copy from the AFCM Canada office or download a new Renewal Application form from the Download options at the left.
Please Note:
Some browsers with built in PDF viewers have trouble with fillable PDF forms. For the best experience, please download the form to your local computer, then open with Adobe Reader. If you don’t have Adobe Reader download it here.
Once you have completed the form, checked ALL the applicable boxes on each page, signed both pages 2 and 3 and included your renewal fees, please send to the AFCM Canada office.
If your are paying by e-transfer (afcmcan@telus.net, no password required) or by Visa or Mastercard, you can send your completed form to us by fax at (604) 539-2043 or by scanning and sending as an e-mail attachment to afcmcan@telus.net.
If paying by cheque, please mail the completed form along with payment to:

Renewal applications not received in good order, without both signatures, all required boxes checked, or payment will be returned for completion.
